• (EAD) Employment Authorization Document
• Driver’s License and/or Passport
• Previous renewal application (if available)

1. For Citizenship Applications:

• Legal Permanent Residence Card “Green Card”
• History of employment, home address(es), trips outside the U.S. for the past five years
• If applicable:
  • • Medicaid Approval Letter
    • SNAP Approval Letter
    • TAMF Approval Letter
